of receiving the referral. We will schedule the patient as soon as they
would like to get in. We have physicians available every day of the week.
Select Pain will also verify and file insurance.
To refer a patient, please fax the online
script to 972-231-7095. Please include the patient’s demographics,
insurance information, clinical information and MRI report, if available.
